If you have Out Of Network benefits we are happy to provide you with a detailed invoice itemizing the payments and codes for your visit.  You can submit that to your insurance company for reimbursement. We do have discounted rates for already-established patients who have Anthem or WHA insurance.

If you are a newly pregnant patient, Premier ObGyn Napa Inc can do your initial visits, up to about 15 weeks, order blood work, arrange for ultrasounds and referrals if needed, but will transfer care to the practice of your choice for later pregnancy management and delivery.

All patients need to fill out a Patient Registration form and Medical History form. Please print and fill out these forms prior to your first visit and bring them with you to shorten your time filling out forms in the office. Even if you have been a patient of Dr. Levy-Gantt’s prior to being seen in this new office, you will still need to fill out new registration and history forms. We apologize for this inconvenience. Thank you for your patience and cooperation in this matter! If you do not fill the forms out ahead of time, please come to your appointment about fifteen minutes early so you can fill them out in the office. Thank you.
